GNU Health MDG 6 and NTDs

Introduction

The United Nations signed in September 2000 the Millennium Declaration, to fight poverty, hunger, disease, illiteracy, environmental degradation, and discrimination against women .

Chaco: El Centro Man...

The Millennium Development Goals (MDG) has derived from the Millennium declaration and it has 8 goals. MDG 6 focuses on HIV/AIDS, Malaria and Tuberculosis.
On the other hand, there is 1 billion (1,000 million) people living with tropical infectious diseases that could represent the “other diseases” part of the MDG 6. This group is named Neglected Tropical Diseases (NTDs).  These are diseases of the poorest, that makes them socially excluded and stigmatized.
GNU Solidario believes that Free Software is a key factor to accomplish these goals. GNU Health, the Free Health and Hospital Information System will help the health centers, researches, NGOs and professionals and volunteers to eradicate these devastating diseases.
What are GNU Health MDG 6 and GNU Health NTDs ?

health_mdg6 is a GNU Health module that incorporates the Millennium Development Goal # 6, to combat HIV/AIDS, malaria and other diseases

GNU Health will incorporate the specific functionality to help prevent, diagnose, treat and control each of these devastating diseases.
The health_mdg6 module will maintain the multi-disciplinary approach, providing tools to the social worker, nurse, biochemist, laboratory , pharmacist, epidemiologist and physician, integrating the health professional into the community.
So, for example, upon a new possible case of active Tuberculosis (TB), GNU Health will execute the following actions : Detect and notify the contacts of the patient; Start profilaxis treatment on contacts ; Create a new laboratory order for culture and antibiogram and check for the strain resistance ; verify the DOTS (Direct Observed Treatment Short) on the patient ; Check the stock for the PPD test-kits and do a forecast for the existing anti-tuberculosis medicaments; Check for availability of special rooms if there is need for hospitalization; inform the health authorities of a new case; start a new prevention campaign.
The health_ntd will cover the Neglected Tropical Diseases.
What diseases will cover GNU Health MDG 6 and NTD?

health_mdg6. The diseases stated in first part the WHO MDG 6:
  • HIV/AIDS
  • Malaria
  • Tuberculosis
health_ntds. Neglected Tropical diseases that affect 1 billion (1000 million) people. Diseases are dynamic by nature, so the list can include emerging diseases at any time, but the initial list will contain :

  • Guinea Worm
  • Lymphatic Filariasis
  • Blinding Trachoma
  • Sleeping Sickness
  • Leprosy
  • Soil Transmitted-Helminths
  • Schistosomiasis
  • River Blindness
  • Chagas
  • Visceral Leishmaniasis
  • Dengue

GNU Health Decision Support on Prescription Writing

Tags

In this entry I will briefly talk about how GNU Health can help the professional in making the best decision, and how to minimize mistakes.
I will focus in prescription writing and how we’re incorporating DS (Decision Support) to GNU Health.
GNU Health uses the WHO (World Health Organization) essential list of medicines by default, so you already have a very nice and updated set for your daily practice.
The medicament model incorporates the pregnancy category. In this case, we have opted for the FDA pregnancy categories, but you can configure GNU Health to use another categorization system.
In the next screenshot, you can see a list of medicines, with an specific color, that specifies the pregnancy category. That visual aid quickly differentiate those medicaments to avoid use in pregnancy (all in categories D and – especially category X ), from those that will be generally safe to use (such as those in categories A and B ).

Color encoded WHO list of essential medicines by FDA pregnancy category
So, from that very starting point, you have GNU Health assisting you visually to choose the right drug.
After you have finished the prescription writing process, GNU Health does different checks in relation to the patient you are writing the prescription.
In its simplest form, let’s check the next example. GNU Health checks the patient sex and age, and determines the risks associated to that patient. In this case, the patient belongs to a potential childbearing / fertile group. So, GNU Health will set a verification flag and will remind the health professional to ask the patient basic but very important questions about the patient status, such as :
– Is she pregnant
– Is the patient planning to become pregnant
– In the case of pregnancy, in which trimester is she in
In other words, GNU Health will stop us from making the prescription until we have verified that what we are prescribing is an educated and thoughtful decision, the best possible for our patient.
By the way GNU Health keeps record of the gynecological and obstetric status of the patient, but this might be the first encounter or her medical record might be in another center that has some dark privative software that does not share the information 😉
GNU Health prescription verification process in specific patients
Of course, the ultimate decision will always be on the doctor. This type of Decision support we aim to minimize common pitfalls due to fatigue, inexperience, etcetera that can jeopardize the wellbeing of the patient.

Many thanks to Dr. Christoph Larsen and Andrew Gledhill (NHS) that have helped me in the process.

We will definitely enhance the Decision Support from GNU Health, such as in the clinical context, always keeping simplicity and just as a guiding / aiding tool for the professional.

GNU Health new partner in India

It’s great to see how fast the GNU Health community is growing, not only in installations, but in companies and institutions delivering implementation and training.
The latest service and training provider is Serpent Consulting Services in India.
“Serpent Consulting Services feels immense pleasure to say that we are in partnership with Thymbra and supporting GNU Health” , said Jay Vora, Managing Director from Serpent CS.
So, now, besides de supporting governments (like Brazil or the European Community ) and multilateral organizations (like United Nations ), there are private companies that are commited to Free Software in health care using GNU Health.
We’ll be generating a map with all the service and training partners in the world, so you can choose the one closest to you.
This raise in the number of service and training providers show that Free Software is both ethical and a great business model.
Thank you to all the health professionals and the community at large that contribute with their talent in suggestions, questions, answers and bug reports.
Finally, our gratitude goes to the GNU project, Richard Stallman and the Free Software Foundation , for adopting GNU Health in their GNU System !
We fight for equity in health care. We deliver health and education with Free Software universally, and it would have been impossible without your contribution.
If you provide services around GNU Health and want your company to be shown in the services section of http://health.gnu.org send us a mail to health@gnusolidario.org and we’ll be happy to publish it.
Remember that, as in today, GNU Health runs on Tryton … just in case 😉
